چکیده

As part of routine prenatal care, the obstetrician of a 25-year-old gravida 1, para 0 woman performed fetal heart-rate monitoring at 22 weeks gestational age. The fetal heart rate was 90 bpm, below the expected range of 120 –160 bpm. This finding prompted a subsequent fetal ultrasound and echocardiogram. The ultrasound exam showed no evidence of hydrops. Cardiac anatomy was normal, with 4 appropriately sized chambers, no valvular defects, and no abnormal communications between the right and left circulations. However, the electrocardiogram demonstrated a 2:1 atrioventricular heart block (1 ventricular beat for every 2 atrial beats). Previously, a firsttrimester screen to detect fetal aneuploidy had been performed and the results were normal. Results of serologic testing for hepatitis B, varicella, and rubella viruses were consistent with maternal immunity. Results of syphilis and HIV antibody tests were also negative. The expectant mother had no significant medical history, was taking no medications, and had no history or symptoms of autoimmune disease.

The use of non-invasive fetal electrocardiography in diagnosing second-degree fetal atrioventricular block

BACKGROUND Complete atrioventricular block in fetuses is known to be mostly associated with autoimmune disease and can be irreversible if no steroids treatment is provided. Conventional methods used in clinical practice for diagnosing fetal arrhythmia are limited since they do not reflect the primary electrophysiological conduction processes that take place in the myocardium. The non-invasive f...

Outcome and growth of infants fetally exposed to heart block-associated maternal anti-Ro52/SSA autoantibodies.

OBJECTIVE The purpose of this work was to analyze outcome with focus on growth in infants fetally exposed to heart block-associated maternal anti-Ro52/SSA autoantibodies and identify maternal factors other than the autoantibodies increasing the risk of fetal heart block. PATIENTS AND METHODS Thirty-two pregnancies in 30 anti-Ro52-positive mothers were included. Seven fetuses developed second-...

Fetal cardiology in the Czech Republic: current management of prenatally diagnosed congenital heart diseases and arrhythmias.

Reliable diagnosis of congenital heart defects and arrhythmias in utero has been possible since the introduction of fetal echocardiography. The nation-wide prenatal ultrasound screening program in the Czech Republic enabled detection of cardiac abnormalities in 1/3 of patients born with any congenital heart disease and up to 83 % of those with critical forms. Prenatal frequency of individual he...

Second-degree Atrioventricular (Mobitz 1) Heart Block in a 52-hour-old Newborn: A Very Rare Case Report

Background: Cardiac conduction disorders are rare syndromes in neonates and children. According to the literature atrial septal defect, especially ostium secundum type, is associated with atrioventricular (AV) block. Wenckebach conduction heart block is very rare in a neonate; however, there is a dearth of research on the prevalence of this type of heart block regarding the neonates and pediatr...

Atrioventricular conduction time in fetuses assessed by Doppler echocardiography.

We performed measurement of mechanical atrioventricular conduction time intervals in human fetuses assessed by Doppler echocardiography and provided reference values. We found that atrioventricular conduction time interval was prolonged with gestational age and decreased with increasing fetal heart rate. No correlation between gestational age and heart rate was found. Using normal limits establ...
